Submit the form that corresponds with the status of your residency and dependency:

U.S. citizen or eligible non-citizen (Dependent): 2020-2021 Request for Re-Evaluation Dependent

U.S. citizen or eligible non-citizen (Independent): 2020-2021 Request for Re-Evaluation Independent 

International, undocumented or DACA: 2020-2021 Alternate Need Determination Form

This form will ask you to estimate your income from January 1, 2021 through December 31, 2021. Follow the instructions on the form carefully. Please do not leave any items blank on the form. You will be asked to describe and document your circumstances.

We understand a loss of income may be causing you anxiety. We will carefully review your circumstances within 10 business days. When the appeal has been reviewed, we will send a communication to the student’s eServices account describing the results.
