If you are a Health Sciences student, your vaccine certificate and records are due 30 days BEFORE your program start date. Even if your specific program requires you to submit a Certificate of Immunization to its office, you will still need to send a separate copy to University Student Health Services.
If you are NOT a Health Sciences student, you have up to 30 days AFTER the semester begins to submit your vaccine documentation.
Flu Vaccine Requirement for Health Sciences Students
Documentation of flu vaccine must be submitted by December 1 or your program's deadline, whichever comes first. If your program starts in January, please submit your current flu vaccine as soon as possible or you may be excluded from clinical settings.
If you receive your flu vaccine through Student Health, you do not need to submit documentation as it is already in our system.
Submit your documentation on time to avoid a hold on your registration!
Students are strongly encouraged to use the Student Health Web Portal to submit immunization records. If you receive an error message when logging into the Portal, please allow up to 5 business days after registering for classes to access the Portal.
You must complete an electronic Certificate of Immunization AND upload a copy of your vaccine records. Otherwise your records will be considered incomplete, and a hold will be placed on your account. Click on the link for detailed .
Please do not submit completed forms multiple times as this delays the data entry process.
