Q: Am I required to get vaccinated?

VCU strongly encourages all full- and part-time faculty and staff to get vaccinated. You are not required to get vaccinated but you are required to complete the employee vaccine survey by July 15.

Q: What is the safety and efficacy of the COVID-19 vaccines?

A: The Centers for Disease Control and Prevention, the Virginia Department of Health and VCU Health have developed frequently asked questions related to the safety and efficacy regarding the types of vaccines available.

Q: Can I get vaccinated on campus?

VCU continues to offer a number of on-campus vaccination opportunities by appointment. Employees can contact VCU Employee Health at (804) 828-0584 to schedule an appointment. If you are not close to campus and need to search for a vaccination opportunity near you, visit

Q: Which vaccine will I receive if I get it through VCU?

The type of vaccine you receive through on-campus clinics will depend on availability. VCU encourages anyone who has not yet been vaccinated to do so as soon as possible with whichever vaccine is is available to you.

Q: What qualifies as acceptable documentation of my vaccine?

A copy of your COVID-19 vaccine card is the preferred form of documentation. However a confirmation email from a health care provider or pharmacy that includes your full name, date of birth, type of vaccine received and vaccination dates is also acceptable.

Q: How do I upload my COVID-19 vaccine documentation?

You can upload your documentation here. You can also find the link in the daily health survey. There are several easy ways for you to upload your documentation:

Q: What happens after I submit my file?

After you hit submit, you will see a confirmation screen that displays what you entered on the form as well as the file you uploaded. If you want to save a copy for your records, please either print or PDF this page or enter your email address at the bottom of the page to receive a copy via email.

At that point, your COVID-19 vaccine documentation will be pending review.

Q: How will I know my submission is verified and I am exempt from surveillance testing and the daily health attestation?

You can view your vaccine status in your myVCU portal under your VCU Profile (click on “Welcome, [eID]” and select VCU Profile from the dropdown menu.) One of the following status messages will be displayed.

If your submission is not approved due to your record being illegible, needing a different file type or if your record is incomplete, please resubmit your file on the form.

If your status says it needs further review, you will be contacted by for next steps.

Q: I already submitted my COVID-19 vaccine documentation. What now?

We are currently reviewing all records submitted to date Check the status of your upload in myVCU portal. The verification process can take up to three days to complete.

Q: If I have already submitted my information, do I need to complete the survey again?

No. As long as you submitted all required documentation (i.e., a record of both shots if you received a two-shot vaccination), you do not need to submit your information again.

Q: Why am I required to report my vaccine status even though I am not required to be vaccinated?

You are only required to respond to the survey. The survey gives you the option to choose to report your vaccination status or decline to answer. If you report that you are not vaccinated, or you choose not to answer, you will need to acknowledge your risk and attest to following safety measures such as wearing a mask, practicing physical distancing, monitoring and attesting to your health daily, and routine surveillance testing.

Q: What should I do if I lose my vaccine record?

The Centers for Disease Control and Prevention recommend taking a photo of both sides of the card as a back up, in case you lose it. But if it is already lost, here are recommendations from the CDC:

Q: If I choose not to get vaccinated but my work responsibilities require me to be in person, what options do I have?

Employees who choose to not be vaccinated must continue to follow safety measures such as wearing a mask, practicing physical distancing, monitoring and attesting to your health daily, and routine surveillance testing.

Q: Who will have access to my vaccine record once I share it?

Only authorized personnel can access the system and vaccine records.

Q: How secure is the system that is used to collect my vaccine record?

VCU will apply reasonable security measures when handling this information and safeguard this information from unauthorized access or disclosure. Please keep in mind that your CDC vaccination card may contain certain sensitive information about you; such as your birth date and possibly patient number. Please consider keeping your card and any images of your card private.

Q: How long is my record valid? Will I be required to get a booster?

This documentation is valid for the fall semester. As additional guidance comes from the Centers for Disease Control and Prevention and the Virginia Department of Health, VCU will determine next steps.
