All VCU employees are required to respond to the employee COVID-19 vaccination survey. While you are not required to disclose your vaccination status to the university, it is required that you respond to this survey. If you have not already done so, please take a couple of minutes to complete the survey now.
Your input assists the university in managing the protocols that support the health and safety of our students, faculty, staff and community.
Your response to this survey informs the level of COVID-19 testing capacity required for symptomatic and asymptomatic testing as well as VCU’s response to evolving pandemic conditions.
The survey completion requirement is for all VCU employees, even those who work 100 percent remotely. The survey provides the option for you to either submit vaccine documentation, indicate that you are in the process of being vaccinated, do or do not plan to be vaccinated, or prefer not to answer. Only authorized personnel review and verify vaccine records. VCU will apply security measures when handling this information to prevent unauthorized access or disclosure. Failure to respond to the survey will result in disciplinary action.
