Request IDS services

In order for a drug study to be initiated with the IDS pharmacy, the following documents are required:

  • Current copy of the protocol
  • Investigational drug brochure, if applicable
  • IRB approval letter
  • FDA 1572, if applicable, or list of authorized prescribers
  • Signed copy of charge estimate letter with valid Banner index code, if applicable
  • Any other pharmacy-related documents, such as the pharmacy binder
  • Study initiation and contact information form

The IDS pharmacy can begin preparation with a minimum of the protocol and, if applicable, the investigational drug brochure. Other documents can be submitted as they are completed. It is advisable to request IDS services as soon as possible, rather than waiting for IRB approval to be granted.

Please note that the IDS pharmacy requires two weeks after receipt of the protocol and investigational drug brochure to prepare adequately for each study. A charge estimate letter for pharmacy services will be sent to the principal investigator and study coordinator once the protocol and study initiation and contact information form have been received.

If any of the above documents are changed or revised during the course of the study, please provide the IDS pharmacy with a copy of the updated information.
