      Obtaining Answer Sheets: Faculty & Instructors

       

      Monroe Park Campus

      Faculty & Instructors can download a PDF copy of the General Purpose Answer sheet on this page. Some notes about printing forms yourself:

      • You can print forms from office-grade, toner-based printers, such as the large, multifunction devices found in most administrative offices:
      • Forms must be double-sided.
      • Forms cannot be photocopied; they must be newly printed.
      • Make sure the print is even and neither too dark nor too light.
      • If the forms are photocopied or poorly printed, they will not scan properly, and you will have to hand-grade your tests.

      If your office printer cannot print the forms at a high quality, you can purchase the printed version of the form from the locations listed below.

      • Barnes & Noble @ VCU
      • Virginia Book Company
      • Wythken Printing (can fill bulk orders and deliver)

      MCV Campus:

      Faculty & Instructors on the MCV Campus have the option of using any of the following test forms:

      • General_Purpose_Answer_Sheet_Sample.pdf
      • 10_Response_Answer_Sheet_Sample.pdf
      • 5_and_26_Answer_Sheet_Sample.pdf

      Faculty & Instructors can request a PDF copy of a test form by emailing gradeit@vcu.edu. Some notes about printing forms yourself:

      • You can print forms from office-grade, toner-based printers, such as the large, multifunction devices found in most administrative offices:
      • Forms must be double-sided.
      • Forms cannot be photocopied; they must be newly printed.
      • Make sure the print is even and neither too dark nor too light.
      • If the forms are photocopied or poorly printed, they will not scan properly, and you will have to hand grade your tests.

      If your office printer cannot print the forms at a high quality, Health Sciences Division instructors can request up to 500 copies of each form at a time at the VCU IT Support Center office in VMI, Room 311.

      GradeIT Usage Agreement

      Printing requirements:

      • Use an office-grade, toner-based printer, such as the large, multifunction devices found in most administrative offices.
      • Print at 100% the same size as distributed to you. Do not shrink the image.
      • Forms must be double-sided (flipping on the long edge of the page).
      • Make sure the print is even and not too dark or light.
      • Print only from this PDF file. Photocopies will not be read correctly by the scanner and will be returned to you for hand grading.

      If your departmental printer cannot print the forms at a high quality, you can alternately obtain the printed version of the form from Barnes & Noble at VCU, Virginia Book Company, as well as Wythken Printing (who fill bulk orders and deliver to departments).

      The PDF file of this form is not to be distributed to students. It is for faculty/staff reproduction only. By downloading this file, you agree to distribute only printed copies to students.
